Memory Care Coordinator

About the role

Responsible for providing memory care services to residents in a senior living facility.

Responsibilities

  • Assess and document cognitive and physical needs of residents.
  • Develop and implement individualized care plans.
  • Provide daily activities and social interactions to enhance quality of life.
  • Monitor and report resident health and safety issues.
  • Collaborate with interdisciplinary team members to ensure comprehensive care.

Requirements

  • Bachelor’s degree in gerontology, psychology, or related field.
  • Minimum 2 years of experience in healthcare or related field.
  • Valid driver’s license and reliable transportation.
  • Ability to pass a background check and criminal history review.

Qualifications

  • Strong interpersonal skills and ability to work effectively with diverse populations.
  • Knowledge of memory care best practices and techniques.
  • Proficiency in Microsoft Office Suite.

Skills

  • Excellent communication and problem-solving abilities.
  • Ability to manage multiple tasks and prioritize responsibilities.
  • Experience with data management systems.
