Member Care Coordinator

Responsibilities

  • Performs comprehensive whole-person assessments to evaluate member medical, behavioral, psychological and social determinants of health needs.
  • Led proactive member engagement initiatives designed to improve health outcomes, strengthen member relationships and support successful navigation of complex healthcare systems.
  • Provides consultative education to members regarding health plan benefits, preventative care opportunities, healthcare options, and available community-based resources to support informed decision-making and self-management.
  • Captures and oversees integrated clinical and non-clinical interventions to ensure alignment with identified member needs, organizational goals and quality outcomes.
  • Evaluates member progress, analyzes barriers to care or engagement and implements appropriate solutions or escalates interventions to improve outcomes and mitigate risk.
  • Directs and coordinates referrals to medical, behavioral health and community-based resources while ensuring continuity and effectiveness of referral.

Qualifications

  • Bachelor’s degree or Associate’s degree in social service, public health, psychology, or related field.
  • 2+ years of experience in customer service, healthcare or social work.
  • Experience working with Electronic Medical Records is preferred.
  • Healthcare or social work experience is preferred.
  • Experience with Motivational Interviewing and Trauma informed care is preferred.
  • Knowledge of Medicare and Medicaid benefits.
  • Proficiency in speaking Spanish, Hmong, or another foreign language is strongly preferred.
  • Valid Driver’s license, verifiable good driving record, and reliable transportation, with the ability to travel, as necessary, to attend off-site meetings on a regular basis.
