This position will be based out of the Emergency Department at Sharp Grossmont Hospital.

Schedule

  • Shift Start Time: 7 AM
  • Shift End Time: 5:30 PM
  • AWS Hours Requirement: 10/40 - 10 Hour Shift
  • Weekend Requirements: As Needed
  • On-Call Required: No

Pay

The stated pay scale reflects the range that Sharp reasonably expects to pay for this position. The actual pay rate and pay grade will be dependent on factors such as years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, and employer business practices.

  • Hourly Pay Range: $42.790 - $55.210 - $61.840 (Minimum - Midpoint - Maximum)

Responsibilities

  • Provides patient/family psychosocial consultation, interventions, and resources addressing Social Drivers of Health (SDOH), family and community support systems, substance use, and protective factors.
  • Works closely with the RN Case Manager/Transition Planner and interdisciplinary team to ensure safe and timely discharge.
  • Maintains required social work credentials in good standing and complies with Sharp Healthcare policies, NASW Code of Ethics, and operational policies.
  • Participates in facility in-service activities to promote understanding of case management and social work processes.
  • Demonstrates ongoing willingness to increase knowledge of social work principles and practices specific to Sharp Healthcare.
  • Represents the facility to healthcare professionals, referral sources, and the public in alignment with Sharp Healthcare’s mission and philosophy.
  • Respects cultural diversity and preserves the basic human rights of all patients and families.
  • Provides input into departmental process improvements and attends monthly departmental meetings, including case review presentations.
  • Completes psychosocial assessments, identifying patient needs related to suicidality, social determinants of health, protective factors, substance use, mental health, family dynamics, and discharge barriers.
  • Documents assessments, interventions, discharge plans, and barriers in the Electronic Medical Record (EMR) per Sharp policies.
  • Collaborates with RN Transition Planner to formulate discharge plans and address barriers, escalating complex cases to leadership as needed.
  • Provides social work consultation, addressing consults related to SDOH, protective factors, family dynamics, end-of-life care, medical decision-making, conserved patients, substance use, and mental illness.
  • Communicates updates to the interdisciplinary team regarding social needs affecting inpatient stay or discharge plans.
  • Attends and documents unit rounds and schedules family conferences for high-risk or difficult social situations.
  • Reports mandated protective issues to appropriate agencies (e.g., Adult Protective Services, Child Welfare Services) per Sharp Healthcare policy.
  • Manages referrals and consults, prioritizing those related to patient safety, and provides community resources to address identified social needs.
  • Ensures continuity of care through hand-offs to team members and evaluates the effectiveness of discharge plans, modifying as necessary.

Requirements

  • Master's Degree in Social Work (Required).

Preferred Qualifications

  • Experience in discharge planning or in a hospital setting.
  • Medical social work experience, including patient/family counseling.
  • California BBS Licensed Clinical Social Worker (LCSW).
