Shift Start Time: 8 AM; Shift End Time: 4:30 PM; AWS Hours Requirement: 8/40 - 8 Hour Shift; Weekend Requirements: As Needed; On-Call Required: No.

Pay

Hourly Pay Range (Minimum - Midpoint - Maximum): $42.790 - $55.210 - $61.840. The stated pay scale reflects the range that Sharp reasonably expects to pay for this position. The actual pay rate and pay grade for this position will be dependent on a variety of factors, including an applicant’s years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, other requirements for the position, and employer business practices.

What You Will Do

The Medical Social Worker provides patient/family psychosocial consultation, inventions and resources which may include, but not limited to, Social Drivers of Health (SDOH), family and community support systems, substance use, and protective factors. The Social Worker works closely with the RN Case Manager/Transition Planner and interdisciplinary team to ensure safe and timely discharge.

Required Qualifications

• Master's Degree in Social Work.

Preferred Qualifications

• Experience in discharge planning or in a hospital setting.
• Medical social work experience including patient/family counseling.
• California BBS Licensed Clinical Social Worker (LCSW) - CA Board of Behavioral Sciences - PREFERRED
