Shift: 1 PM – 11:30 PM, 10-hour shifts (10/40 schedule). Weekend work as needed; no on-call requirement.
Pay
Hourly pay range: $42.79 – $61.84. The stated scale reflects the range Sharp reasonably expects to pay; actual rate and pay grade depend on experience, skills, education, internal alignment, market factors, and employer business practices.
Responsibilities
- Provide consultation, psychosocial counseling, and support services to Sharp HealthCare patients and families.
- Assess psychosocial needs, complete assessments, and identify key clinical issues (grief, loss, adjustment to illness, psychiatric concerns, developmental milestones, life transitions).
- Document assessment findings, treatment plans, interventions, and progress in medical records per department and regulatory guidelines.
- Consult with multidisciplinary team members, attend rounds, and communicate patient status and follow-up needs.
- Arrange and participate in patient care conferences for high-risk psychosocial cases.
- Utilize and refer patients to internal and community resources to facilitate timely discharge and ongoing support.
- Consult and intervene in crisis situations; assist staff in de-escalating volatile situations and educate on Code Green protocols.
- Coordinate and facilitate critical incident debriefings; develop nursing education to enhance staff knowledge.
- Act as a patient advocate: encourage family participation in decision-making, report mandatory protective concerns (elder/child abuse, domestic violence), and maintain patient privacy and confidentiality.
- Develop and implement treatment goals addressing identified needs; practice boundary setting, cultural sensitivity, and crisis intervention.
- Utilize appropriate treatment modalities (individual, conjoint, group) and seek resources for unusual or new situations.
- Evaluate patient/family response to interventions and revise care plans as needed.
Requirements
- Master’s Degree in Social Work (required).
- California BBS Licensed Clinical Social Worker (LCSW) (preferred).
- Hospital experience (preferred).
