Medical Social Worker - Hospice - Ruffin Rd - Full Time - Day Shift

Shift: 8:00 AM – 4:30 PM, 8-hour shift / 40-hour work week. Weekend work as needed; no on-call requirement.

Pay

The stated pay scale reflects the range Sharp reasonably expects to pay for this position: $42.79 – $55.21 – $61.84 per hour. Actual pay rate and pay grade will depend on years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, and employer business practices.

Responsibilities

• Provide consultation, psychosocial counseling, and support services to Sharp HealthCare patients and families.
• Mobilize patients’ and families’ personal and environmental resources to maximize benefits of medical care and achieve optimal personal and family functioning.
• Set daily priorities and revise schedule based on patient/family needs; respond to crises in a timely manner.
• Maintain contact with patients and families according to department guidelines, need, and plan of care.
• Deliver counseling and education on coping, anticipatory grief, urgent caregiver/safety issues, and end-of-life concerns.
• Promote patient-centered care, autonomy, and advocacy; identify and refer to additional community resources as needed.
• Conduct comprehensive psychosocial assessments within five days of service start and throughout care, covering:
  • Goals of care
  • Support network
  • Safety issues
  • Caregiver status and long-term care plan
  • Financial, legal, and insurance issues
  • Bereavement risks
  • Family dynamics and coping
  • Final arrangements
  • Bereavement needs upon death
• Document professionally and appropriately, including initial assessments, routine visits, clinical team communications, and discharges; complete documentation by end of shift.
• Develop, revise, and document plan of care based on client needs; perform services as ordered; update visit frequency based on patient/family need and clinical judgment.
• Clarify and perform within professional scope of practice; understand roles of interdisciplinary team members and refer appropriately.
• Maintain ongoing communication with team members; demonstrate respectful, professional behavior with employees, patients, families, and community partners.
• Practice culturally competent care and sensitivity to cultural differences.

Requirements

• Master’s Degree in Social Work.
• Valid California Driver’s License.
• AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) certification.
• Reliable transportation and adequate personal insurance coverage.
• Clean driving record in accordance with employer DMV pull-notice program and Sharp HealthCare Driver Guidelines.

Preferred Qualifications

• Hospital or medical experience.
• California BBS Licensed Clinical Social Worker (LCSW) credential.

• Excellent communication, problem-solving, and documentation skills.
