About the Role
The Social Worker functions as an active member of the health team under the direction of a physician, in accordance with the physician's orders and the written plan of care. This role provides patient services aligned with the hospice philosophy, assists the Interdisciplinary Team in understanding significant social, emotional, and cultural factors related to terminal illness, conducts psychosocial assessments and evaluations, and provides emotional counseling to patients and families.
Qualifications
- Master’s Degree in Social Work from a school accredited or approved by the Council on Social Work Education
- ACSW or LCSW preferred but not required
- Clinical experience relevant to the psychosocial needs of patients and families
- Valid driver’s license and auto insurance
Core Competencies
- Knowledge of psychosocial theories and use of psychosocial treatment techniques
- Understanding of cultural beliefs and practices related to death and dying
- Excellent oral and written communication skills
- Ability to work compassionately with dying patients and their families
- Sympathetic attitude and positive communication skills when interacting with the hospice team
- Proficiency in Microsoft Windows 10 operating system
