Full Time Hospice Social Worker

Job Duties

• Visits patients wherever they reside, including private homes, skilled nursing facilities, assisted living facilities, and hospitals.
• Completes a psychosocial assessment on each assigned patient in a timely manner and documents in the electronic medical record.
• Utilizes community resources for patients, such as in-home services, Meals on Wheels, etc.
• Aids the patient, family, and caregiver in coping with issues related to death and dying and provides grief support as needed.
• Provides information about hospice services and completes hospice admission paperwork.

Qualifications

• Graduated from a School of Social Work approved by the Council of Social Work Education with a Master's Degree (preferred).
• Bachelor's Degree in Social Work candidates will be considered.

Work hours are primarily Monday through Friday, 8am to 5pm, with flexibility with scheduling and hours a must.

Hourly rate, mileage reimbursement provided. Compensation is based on years of experience.

Computer skills for charting/EMR, basic use of Word, Excel, Email; smart phone for mileage apps, etc.

A loving compassionate heart is a must.
