About the role

The Medical Respite Program Coordinator/Case Manager is responsible for providing comprehensive care and support to individuals in need of respite services.

Responsibilities

  • Assess and document patient needs and develop individualized care plans
  • Coordinate with healthcare providers to ensure continuity of care
  • Maintain accurate records and reports
  • Provide emotional support and counseling to patients and families
  • Conduct outreach and community engagement activities
  • Participate in staff meetings and training sessions

Requirements

  • Bachelor’s degree in a related field
  • At least 2 years of experience in case management or related field
  • Valid driver’s license and access to reliable transportation
  • Excellent communication and interpersonal skills
  • Ability to work flexible hours including evenings and weekends

Qualifications

  • Knowledge of medical terminology and procedures
  • Experience with electronic health record systems
  • Strong organizational and time management skills

Skills

  • Strong written and verbal communication skills
  • Ability to handle sensitive and confidential information
  • Proficiency in Microsoft Office Suite
