About the role
Mentor Community Services, a part of the Sevita family, provides community-based services for individuals with intellectual and developmental disabilities. The Program Clinical Coordinator (Case Management) works at the California MENTOR Family Home Agency Office in Alhambra, serving clients in East LA, Whittier, Pico Rivera, and Montebello.
Responsibilities
- Coordinate the development, review, and implementation of services, case management, behavior intervention plans, and treatments provided to individuals served.
- Oversee all services provided to individuals and their families, monitor quality of care delivered, ensure compliance with contractual terms, and monitor regulatory program compliance.
- Aid in the development of quality services and engaging activities that meet the individual served needs.
- Build and maintain relationships with families and external case managers.
Requirements
- Bachelor’s degree in human services or related field.
- One year of work-related experience working with individuals with intellectual and developmental disabilities.
- Will consider candidates without Bachelor's degree if they meet these requirements: High School diploma/GED, at least two years of experience in a similar program serving individuals with developmental disabilities, Valid California driver's license in good standing, Reliable vehicle with proof of valid registration and insurance, A reliable, responsible attitude and a compassionate approach and a commitment to quality in everything you do.
- On-call availability required after hours and on weekends.
Qualifications
- Bachelor’s degree in human services or related field.
- One year of work-related experience working with individuals with intellectual and developmental disabilities.
- Valid California driver's license in good standing.
- Reliable vehicle with proof of valid registration and insurance.
- A reliable, responsible attitude and a compassionate approach and a commitment to quality in everything you do.

Pay
$45,800 annual salary (overtime eligible).
Schedule
Full-time position, schedule Monday through Friday from 8:30 AM to 5:30 PM.
