Medical Residential LPN

About the role

The Medical Residential LPN position at Sertoma Center, Inc. is responsible for the health and welfare of the people supported in the medical residential group home and provides a family-type home atmosphere. The role involves managing all activities including upkeep of the home, food budgeting, and acting as part of a team in emergency situations.

Responsibilities

• Ensure health and safety of people supported by ensuring adequate exercise, providing assigned nursing services, maintaining up-to-date medical/emergency information, ensuring home is hazard-free, participating in fire evacuation training, maintaining a clean and sanitary environment, and promptly reporting any maintenance needs.
• Create a family atmosphere through interaction, counseling, outings, conversation, and encouraging people supported to follow dietary menus.
• Meet the needs of people supported through training, maintaining a clean home, meal preparation, assisting with personal hygiene, and shopping.
• Work on training actions and BSP actions/objectives (when indicated) as outlined in program plans with people supported on a regular basis.
• Document as specified and meet monthly with the case manager on these and other issues.
• Participate in monthly house meetings and attend circle of support meetings, monthly staff meetings, and in-service meetings.
• Encourage people supported to follow dietary menus and post all menus/special diets for easy access for other staff.
• Plan and follow through with regularly scheduled recreational activities at the house.
• Work regular schedule, participate as a team player, offer to cover shifts when needed, and distribute medications and other medical treatments as prescribed.
• Help residents cash paychecks, counsel and assist them with financial/spending skills, and attend circle of support meetings, monthly staff meetings, and in-service meetings.
• Maintain the vehicle logs to include regular cleaning, monthly mileage form, gas receipts, and promptly report any vehicle problems to the maintenance department.
• Communicate with the appropriate staff immediately regarding any concerns, problems, or outside issues from external relationships.
• Complete and document nursing notes daily.

Qualifications

• Current unencumbered LPN license
• Previous experience with ID/DD population preferred but not required
• Time management skills and ability to multi-task are beneficial in this position

Skills

• Physical requirements: Sight and hearing, ability to lift/assist people supported and to intervene during behavioral episodes in a manner in keeping with company policies
