Responsibilities
- Serves as a Clinical Partner supporting the mission, vision, and core values of HarmonyCares.
- Assists in patient care coordination, including preparing patients for examinations and treatments.
- Coordinates daily travel itineraries and transports clinicians to scheduled appointments.
- Prepares and secures patient consent forms, records accurate patient care documentation, and coordinates care as directed.
- Performs phlebotomy, EKGs, or other procedures as requested by clinicians, ensuring patient confidentiality and maintaining compliance with relevant regulations.
- Updates patient care plans and SDoH assessments in the electronic medical record.
- Maintains supplies and equipment for treatment and ensures proper vehicle maintenance.
- Attends required meetings and in-services.
Qualifications
- High school diploma or GED equivalent.
- Graduated from an accredited Medical Assistant, Phlebotomy, EMT, or related medical education/training program, or one year of experience as a Medical Assistant or Phlebotomist.
- Current CPR certification meeting company guidelines.
- Knowledge of medical terminology and basic computer literacy, including EMR, iPad, and GPS.
- Strong interpersonal communication and customer service skills.
- Valid driver’s license and good driving record.
