Med Care Manager (LPN/RN)

Role Overview

The LPN Medication Nurse provides licensed nursing support in a senior living environment, ensuring safe medication administration, health monitoring, and accurate clinical documentation.

Key Responsibilities

• Administer medications and treatments according to physician orders
• Follow the Six Rights of Medication Administration to ensure safety
• Document medication administration accurately in MAR/TAR in real-time
• Monitor symptoms, administer medications for relief, and report adverse reactions or refusals promptly
• Conduct Service & Health Updates (SHUs) and support care plan updates
• Observe and report physical, emotional, or cognitive changes in residents
• Maintain medication carts, complete narcotic counts, and ensure compliance with regulations

Qualifications & Skills

• Active LPN license in good standing
• CPR and First Aid certification
• Strong assessment and documentation skills
• Experience in senior care or medication administration preferred
