MDS Nurse (RN/LVN)

Key Responsibilities

• Complete and submit accurate, timely MDS assessments (MDS 3.0) for both skilled (Medicare) and long-term care residents
• Set, track, and manage Assessment Reference Dates (ARDs)
• Ensure documentation supports clinical conditions, including wounds and complex care needs
• Conduct and complete Care Area Assessments (CAAs)
• Collaborate with the interdisciplinary team on care planning and resident outcomes
• Review clinical documentation for accuracy, compliance, and reimbursement optimization
• Maintain up-to-date knowledge of Medicare/Medicaid guidelines and regulatory requirements
• Support survey readiness and participate in audits as needed

Qualifications (Required For Consideration)

• Active RN or LVN license in the state of Texas
• Proven experience completing MDS assessments in a skilled nursing or long-term care setting
• Experience completing skilled (Medicare) MDS assessments
• Experience with long-term care residents and wound-related MDS coding/documentation
• Strong knowledge of: MDS 3.0 process, RAI (Resident Assessment Instrument) Manual, Care Area Assessments (CAAs), Medicare/Medicaid reimbursement guidelines
• Strong attention to detail and organizational skills

Preferred Experience

• 1–2+ years of experience in a skilled nursing facility (SNF/LTC)
• Experience with wound care, rehab, or case management
• Familiarity with PointClickCare (PCC) or similar EMR systems
