Key Responsibilities
- Complete and submit accurate, timely MDS assessments (MDS 3.0) for both skilled (Medicare) and long-term care residents
- Set, track, and manage Assessment Reference Dates (ARDs)
- Ensure documentation supports clinical conditions, including wounds and complex care needs
- Conduct and complete Care Area Assessments (CAAs)
- Collaborate with the interdisciplinary team on care planning and resident outcomes
- Review clinical documentation for accuracy, compliance, and reimbursement optimization
- Maintain up-to-date knowledge of Medicare/Medicaid guidelines and regulatory requirements
- Support survey readiness and participate in audits as needed
Qualifications (Required For Consideration)
- Active RN or LVN license in the state of Texas
- Proven experience completing MDS assessments in a skilled nursing or long-term care setting
- Experience completing skilled (Medicare) MDS assessments
- Experience with long-term care residents and wound-related MDS coding/documentation
- Strong knowledge of: MDS 3.0 process, RAI (Resident Assessment Instrument) Manual, Care Area Assessments (CAAs), Medicare/Medicaid reimbursement guidelines
- Strong attention to detail and organizational skills
Preferred Experience
- 1–2+ years of experience in a skilled nursing facility (SNF/LTC)
- Experience with wound care, rehab, or case management
- Familiarity with PointClickCare (PCC) or similar EMR systems
