About the role
Marin Post Acute is seeking a dedicated Full-Time LVN MDS Coordinator to join our clinical leadership team. In this essential role, you'll oversee the MDS process, support regulatory compliance, optimize reimbursement, and collaborate with our interdisciplinary team to ensure exceptional resident care and outcomes.
Responsibilities
- Coordinate and manage the MDS/RAI process in accordance with CMS, federal, and state regulations
- Complete accurate and timely MDS assessments and supporting documentation
- Develop and update comprehensive resident care plans
- Collaborate with nursing, therapy, dietary, and interdisciplinary team members
- Monitor PDPM documentation and reimbursement accuracy
- Track quality measures and identify opportunities for clinical improvement
- Assist with survey preparation, audits, and regulatory compliance initiatives
- Educate and support staff regarding MDS processes and documentation requirements
- Partner with facility leadership to ensure clinical and operational excellence
Requirements
- Current, active California LVN license
- Minimum 1 year of MDS experience in a Skilled Nursing Facility (SNF) or Post-Acute setting
- Knowledge of MDS 3.0, RAI process, PDPM, Medicare, and Medicaid reimbursement
- Strong assessment, documentation, and organizational skills
- Ability to work independently while collaborating with interdisciplinary teams
Preferred
- RAC-CT certification
- Experience with PointClickCare
- Prior skilled nursing facility leadership experience
