MDS Nurse - LVN

About the Role

The Care Management Specialist coordinates the Resident Assessment Instrument (RAI) process to ensure accurate, timely assessments aligning with Medicare, Medicaid, OBRA, and other payer requirements. They oversee resident evaluations, validate documentation, and collaborate with interdisciplinary teams to develop effective care plans.

Responsibilities

• Ensure comprehensive assessments of residents' health and functional status.
• Participate in pre-admission procedures and ensure proper documentation for reimbursement.
• Manage MDS assessments, including PPS, quarterly, and significant change reviews, ensuring compliance with guidelines.
• Track case-mix documentation, manage default days, and rectify assessment issues.
• Conduct audits, review validation reports, and ensure timely electronic submission of MDS data.
• Collaborate with clinical teams and external case managers; support staff training on RAI processes.
• Maintain organized data records, stay updated on reimbursement regulations, and support departmental operations.

Requirements

• Licensed registered nurse (RN or LVN) in the applicable state; RN preferred for specific functions.
• Minimum two years healthcare experience, with familiarity in MDS completion and case management preferred.
• Strong clinical assessment, documentation, interpersonal, and communication skills.
• Proficiency in office and MDS-related software applications.
• Ability to lead interdisciplinary teams and work flexibly as needed.
