MDS Nurse (RN or LVN)

About the role

Marin Post Acute is seeking an experienced and detail-oriented MDS Coordinator (RN or LVN) to join their dedicated clinical leadership team. This role oversees the MDS process, ensures regulatory compliance, optimizes reimbursement, and supports exceptional resident outcomes.

Responsibilities

• Cook up and manage the MDS/RAI process in line with federal, state, and CMS regulations
• Accurately and promptly assess, plan, and document MDSs and care plans
• Collaborate with the interdisciplinary team (IDT) to craft individualized, resident-centric care plans
• Keep tabs on and enhance quality measures, compliance standards, and reimbursement accuracy
• Pass on education and guidance to nursing staff about MDS processes and regulatory updates
• Support audits, reporting, quality improvement initiatives, and survey readiness activities
• Partner with facility leadership to support clinical excellence and operational success

Qualifications

• A current and active California RN or LVN license
• One year of MDS/RAI experience in a skilled nursing or post-acute setting preferred
• A strong grasp of PDPM, CMS reimbursement systems, care planning, and regulatory compliance
• Outstanding organizational, analytical, and documentation skills
• The ability to work collaboratively with clinical, administrative, and leadership teams
• A commitment to delivering high-quality resident care and positive outcomes
