About the role
Marin Post Acute is seeking an experienced and detail-oriented MDS Coordinator (RN or LVN) to join their dedicated clinical leadership team. This role oversees the MDS process, ensures regulatory compliance, optimizes reimbursement, and supports exceptional resident outcomes.
Responsibilities
- Cook up and manage the MDS/RAI process in line with federal, state, and CMS regulations
- Accurately and promptly assess, plan, and document MDSs and care plans
- Collaborate with the interdisciplinary team (IDT) to craft individualized, resident-centric care plans
- Keep tabs on and enhance quality measures, compliance standards, and reimbursement accuracy
- Pass on education and guidance to nursing staff about MDS processes and regulatory updates
- Support audits, reporting, quality improvement initiatives, and survey readiness activities
- Partner with facility leadership to support clinical excellence and operational success
Qualifications
- A current and active California RN or LVN license
- One year of MDS/RAI experience in a skilled nursing or post-acute setting preferred
- A strong grasp of PDPM, CMS reimbursement systems, care planning, and regulatory compliance
- Outstanding organizational, analytical, and documentation skills
- The ability to work collaboratively with clinical, administrative, and leadership teams
- A commitment to delivering high-quality resident care and positive outcomes
