About the Role
Responsible for creating an exceptional HCA Healthcare at Home customer experience by providing support to clinical liaisons, therapists, and other associates. Ensures delivery of quality care with timely scheduling and follow-up for rehab needs. Maintains the objectives, goals, and philosophy of healthcare delivery in conjunction with HCA Healthcare at Home’s mission. Partners with agency clinical leadership to monitor efficiency levels within operations, ensuring prudent financial resource utilization. Ensures efficient and effective communication among community leadership and the interdisciplinary HCA Healthcare at Home team.
Responsibilities
- Supports clinical liaisons and the rehabilitation team to ensure consistent service delivery of the highest quality care for patients and families.
- Assists in identifying clinical program needs and supports implementation with community leadership and Clinical Liaisons.
- Meets consistently with Clinical Liaisons to monitor customer experience, quality of care delivery, and facilitate program growth.
- Works closely with Clinical Managers and provides rehab expertise on delivering quality care for patients and families.
- Maintains involvement in the interview process for field-level clinicians.
- Supports the on-boarding process of HCA Healthcare Services rehab associates.
- Serves as an educational resource to the community regarding healthcare services.
- Coordinates available resources for staff and families.
- Markets HCA Healthcare at Home to appropriate audiences.
- Participates in the continuous quality improvement program as required.
- Supports effective continuous quality improvement and Outcome Based Quality Improvement (OBQI).
- Ensures regulatory compliance of any certified Rehab Agency sites.
- Monitors and directs effective utilization of HCA Healthcare at Home documentation standards and expectations.
- Analyzes Key Performance Indicators and problem-solves to ensure quality patient care and outcomes.
- Reports on trends and activities using defined reporting processes and assists with action plans to correct, improve, or advance identified trends.
- Works in conjunction with local HCA Healthcare at Home Skilled Nursing Communities and Hospice & Family Care agencies to ensure appropriate utilization, regulatory compliance, and clinical outcomes for patients.
- Maintains current knowledge of legislative, community, and payment regulations that affect agency goals and performance.
- Adheres to established confidentiality standards and ensures standards are maintained by all colleagues.
- Ensures compliance with all Infection Control, Universal Precautions, and OSHA standards for healthcare professionals.
- Ensures agencies maintain a safe and clean work area and adhere to community and corporate safety standards.
Requirements
- Bachelor’s Degree in a therapy-related program required.
- Two years of multi-site and/or multi-product line supervisory experience preferred.
- Current state licensure and/or minimum qualifications preferred for a therapy position (e.g., Occupational Therapist or Licensed Physical Therapist).
- Travel is required; must have reliable transportation with current insurance coverage and a valid driver’s license.
