Duties and Responsibilities
- Counsels and assesses specialized care needs for people living with ALS and caregivers.
- Manages a portfolio of clients, ensuring their needs are met within The ALS Association's capabilities. Oversees care services staff in an assigned territory to ensure clients and caregivers receive necessary programs and services.
- Provides information on programs, services, resources, and educational materials. Facilitates referrals to and partnerships with healthcare providers and community agencies.
- Coordinates support group activities and evaluates their effectiveness. Collaborates with the CS Director and Managing Director to implement program outcomes and evaluation processes.
- Maintains quarterly contact and engagement with people living with ALS and/or their caregivers. Documents all pertinent engagements according to Care Services Standards.
- Educates and informs the public and professionals about ALS, its management, related research, and The ALS Association's mission.
- Serves as a liaison between the ALS Clinic multidisciplinary team and The Association, developing and maintaining clinic relationships through ongoing interactions.
- Develops and delivers professional in-service educational programs to staff and healthcare professionals.
- Participates in continuing education seminars, workshops, and conferences.
- Mentors new staff members.
Qualifications
- Bachelor’s Degree or related experience in social work, case management, health administration, allied and public health, nursing, or a related field required.
- Minimum of four years of related professional experience and/or training, or equivalent combination of education and experience.
- Demonstrated management skills including problem-solving, solid reasoning, critical thinking, budget/program planning, performance improvement, organizational planning, implementing, and monitoring performance standards.
- Knowledge of ALS is preferred but not required.
- Experience with progressive neurodegenerative diseases is preferred.
- Knowledge of Medicare, Medicaid, patient assessments, planning, and case management is preferred.
The ALS Association's pay range for this position is $50,649 - $79,346. The Association may require employees to be fully vaccinated against COVID-19, as considered per CDC guidelines, with the option for legally required exemptions.
