Manager of Care Management Services
Mercy First is a not-for-profit human and social service agency serving children and families in need since 1894. Today, we address the emotional and physical needs of children and families in Brooklyn, Queens, Staten Island, and across Long Island through innovative treatments and life-changing interventions. Our services include community-based prevention, family foster care, group homes, and short-term residential services for unaccompanied migrant children. Each year, we help over 3,000 children, teenagers, and families overcome obstacles and develop their full potential.
As a member of the Collaborative for Children and Families Health Home, Mercy First serves youths aged 0 to 20. Our Care Management program is guided by core values: family-driven and youth-guided services, individualized care, community-based supports, evidence-based and trauma-informed practices.
About the role
The Care Management Supervisor oversees the care coordination team, ensuring all care management supports and services are provided to assigned staff working with children, youth, their birth families, and foster families. This role is critical in achieving coordination, integration, and partnership in delivering care to children and families with complex needs.
Responsibilities
- Overall responsibility for overseeing all aspects of assigned cases.
- Hire, train, supervise, and evaluate all assigned staff.
- Coordinate monthly staff meetings.
- Assist caseworkers in developing realistic treatment goals for children on their caseloads.
- Ensure assigned staff conduct field visits to clients’ homes, schools, treatment centers, etc.
- Ensure monthly foster home visits to assess quality of care, well-being, and safety.
- Ensure required/recommended services (medical, developmental, clinical, educational) for each child and monitor outcomes and progress.
- Ensure required documentation (progress notes, FASPs, Permanency Reports, Plan Amendments) is completed within established timeframes.
- Attend ACS and agency staff meetings and trainings.
- Mentor new staff, providing shadowing opportunities and assistance with paperwork.
- Respond to emergencies as needed and/or directed.
- Attend to budgetary processes.
- Maintain program statistics and prepare reports.
- Represent the program at relevant community and professional organizations.
Requirements
- Bachelor’s degree in social work or related field.
- 3+ years’ experience as a Care Manager.
- Knowledge of Medicaid Health Homes Comprehensive Care Management.
- Knowledge of and/or training in child development stages and family dynamics.
- Excellent communication skills (verbal and written).
- Proficiency in Microsoft Word and Outlook.
- Ability to engage with clients.
- A valid driver’s license is strongly preferred.

Pay
$61,000–$68,000 per year, plus a $1,000 hiring incentive after 500 worked hours.
