About the role
Provide complex, high-quality care to infants, children, and adults in their homes across multiple counties. Support the transition of care from hospital to home for clients with rare and complex diseases.
Responsibilities
- Deliver direct patient care in the home setting
- Provide extensive training to ongoing care teams
- Ensure client safety and well-being throughout the transition process
Requirements
- Prior experience in NICU, PICU, ICU, or home care with trach/vent patients
- Pediatric trach/vent expertise
- Current CPR certification
- Ability to pass high-tech adult and pediatric simulations
- Flexibility to work overnight shifts and weekends
- Willingness to travel within designated service areas
