Home Health LPN - Transition Team
About the role
Available at Maxim Healthcare, the Licensed Practical Nurse (LPN) – Transition Team position is a supplemental care unit focused on supporting patients transitioning home. This team is deployed immediately after a patient referral and provides care for up to 90 days. Upon transition, nurses move on to support the next new admission.
Responsibilities
- Utilize the nursing process to assess, plan, implement, and evaluate patient care
- Initiate and update plans of care as directed; request provider revisions as patient needs change
- Provide therapeutic, preventive, and rehabilitative nursing interventions based on diagnosis and goals
- Respond to changes in patient condition and report to physicians or supervisors as appropriate
- Administer prescribed medications and treatments and document patient response
- Identify opportunities for patient/family education, care coordination, goal setting, and discharge planning
Requirements
- Current Licensed Practical Nurse (LPN) license in the state of practice
- Current Health Certificate (per contract or state regulation)
- Current PPD or Chest X-Ray
- Current BLS certification
- Ability to travel 45 minutes to an hour
- Ability to work with pediatrics and adults
- Ability to care for patients with varied acuity (I.e. g-tube, trach, vent)
- One year of LPN experience preferred
Qualifications
Available to work full-time and part-time, this role requires a current LPN license, health certificate, and BLS certification. Travel requirements and patient care abilities are also essential.
Skills
Strong communication skills, ability to work independently, and a compassionate approach to patient care are key.

Pay
Hourly Pay: $37.50 - $40.50 / per hour
