Responsibilities

  • Affirms patients in navigating post-acute care
  • Develops patient-centered transition plans using evidence-based guidelines
  • Engages in strategic relationships with healthcare providers and organizations

Qualifications

  • Graduate of an approved nursing, therapy, or social work program
  • Licensed in the state of practice
  • Two years of field experience
  • Experience with federal, state, and local healthcare regulations
  • Basic technology skills, including mobile device operation
  • Prior experience in home health or healthcare sales is preferred

Requirements

  • Valid state driver license
  • Automobile liability insurance
  • Dependable transportation in good working condition
  • Safety driving ability in all weather conditions
