Responsibilities
- Aids patients in navigating post-acute care processes.
- Develops patient-centered transition plans using evidence-based clinical guidelines and preventative measures.
- Engages in strategic relationships with health systems, hospitals, inpatient facilities, physicians, and executive-level opportunities.
Qualifications
- Graduate of an approved school of nursing, therapy, or social work.
- Licensed in the state where practicing.
- Two years of demonstrated field experience.
- Experience and understanding of relevant federal, state, and local laws and regulations.
- Basic technology skills, including operation of a mobile device.
- Preferred: LPN or PTA, three years of field experience, previous home health or healthcare sales experience.
Requirements
- Valid state driver license.
- Automobile liability insurance as required by law.
- Dependable transportation in good working condition.
- Safety and ability to operate a vehicle in various weather conditions.
