Responsibilities
- Affirms patients in navigating post-acute care
- Develops patient-centered transition plans using evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics
- Represents area branches in strategic relationships with health systems, hospitals, inpatient facilities, physicians and physician groups, and executive level opportunities
Qualifications
- Must be a graduate of an approved school of nursing, therapy or social work
- Must be licensed in the state where they currently practice
- Must have two years' demonstrated field experience
- Must have demonstrated experience and understanding of federal, state, and local laws and regulatory guidelines governing the operations of Medicare certified home health and hospice
- Must have basic demonstrated technology skills, including operation of a mobile device
- LPN or PTA preferred
- Three years of field experience is preferred
- Previous experience in home health or healthcare sales is preferred
Requirements
- Must possess a valid state driver license
- Maintain automobile liability insurance as required by law
- Maintain dependable transportation in good working condition
- Must be able to safely drive an automobile in all types of weather conditions
