Responsibilities

• Affirms patients in navigating post-acute care
• Develops patient-centered transition plans using evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics
• Represents area branches in strategic relationships with health systems, hospitals, inpatient facilities, physicians and physician groups, and executive level opportunities

Qualifications

• Must be a graduate of an approved school of nursing, therapy or social work
• Must be licensed in the state where they currently practice
• Must have two years' demonstrated field experience
• Must have demonstrated experience and understanding of federal, state, and local laws and regulatory guidelines governing the operations of Medicare certified home health and hospice
• Must have basic demonstrated technology skills, including operation of a mobile device
• LPN or PTA preferred
• Three years of field experience is preferred
• Previous experience in home health or healthcare sales is preferred

Requirements

• Must possess a valid state driver license
• Maintain automobile liability insurance as required by law
• Maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
