Home Health Registered Nurse - Transition Team RN

About the role

Available at Maxim Healthcare, the Transition Team is a supplemental care unit of highly skilled nurses focused on supporting patients transitioning home while permanent nursing staff are recruited.

Responsibilities

• Utilize the nursing process to assess, plan, implement, and evaluate patient care
• Initiate and update plans of care as directed; request provider revisions as patient needs change
• Provide therapeutic, preventive, and rehabilitative nursing interventions based on diagnosis and goals
• Respond to changes in patient condition and report to physicians or supervisors as appropriate
• Administer prescribed medications and treatments and document patient response
• Identify opportunities for patient/family education, care coordination, goal setting, and discharge planning

Requirements

• Current Registered Nurse (RN) license in the state of practice
• Current Health Certificate (per contract or state regulation)
• Current PPD or Chest X-Ray
• Certified Basic Life Support (BLS) certification
• Able to travel 45 minutes to an hour
• Able to work with pediatrics and adults
• Able to care for (training available) patients with varied acuity (I.e. g-tube, trach, vent)
• One year of RN experience preferred

Pay

Hourly Pay: $38.50 - $41.50 / per hour
