LPN - Transitional Care Team

About the role

We have immediate opportunities on our Transition Team for Licensed Practical Nurses (LPNs) interested in helping infants, children, and adults receive complex care at home in Philadelphia, PA. This team supports the transition of care from the hospital or from our HICU® Health Services team to ongoing home care services. This role offers a unique opportunity to partner with CHOP and learn how to care for clients with rare and complex diseases. As part of the Transition Team, you will deliver quality care to clients and provide extensive training to the ongoing care team.

Present openings are for days and overnight shifts Monday through Sunday.

Responsibilities

• Deliver quality care to clients with complex medical needs
• Provide extensive training to the ongoing BAYADA care team
• Work in a teaching setting to prepare and educate care teams
• Travel throughout Philadelphia County as needed

Requirements

• Current license as a Licensed Practical Nurse (LPN)
• NICU, PICU, ICU, or home care pediatric trach/vent experience required
• Achieve a passing score on high-tech Adult and Pediatric simulations
• Hold current CPR certification
• Ability to commit to an ongoing, consistent schedule

Pay

LPNs at BAYADA earn a salary of $30–$42 per hour. Transition Team rates: $40–$42. Standard rates: $30–$35. Rate is based on qualifications and experience.

Schedule

Openings available for days and overnight shifts, Monday through Sunday. Full-Time and Part-Time positions offered.
