LPN Supervisor Evening Shift-Assisted Living

Supervise daily nursing activities of the facility during the assigned shift in accordance with federal, state, and local standards, guidelines, and regulations to ensure the highest degree of quality care is maintained.

Responsibilities

• Conduct daily nursing functions in accordance with current rules, regulations, and guidelines.
• Ensure nursing personnel comply with written policies and procedures, including the safety program.
• Admit, transfer, and discharge residents with appropriate documentation.
• Participate in the orientation of new residents and family members.
• Make rounds with healthcare providers and utilize nursing personnel based on resident care plans and provider orders.
• Address and resolve issues with residents and/or family members; report significant issues to the supervisor.
• Participate in facility surveys (inspections) as requested by management.
• Oversee clinical programs such as dementia care, wound care, quality assurance, infection control, and admissions.
• Complete initial assessments and develop interim care plans; update and review care plans as needed.
• Communicate with healthcare providers regarding treatment plans, orders, and progress notes.
• Review resident medical records for treatments, medication orders, diets, and labs for implementation and follow-up.
• Requisition and arrange diagnostic and therapeutic services as ordered by healthcare providers.
• Communicate department policies and procedures to personnel, residents, and visitors.
• Provide written (24-hour report) and oral reports concerning resident condition changes and shift activities to nursing management.
• Complete and file accident/incident reports and submit them to the Director of Nursing.
• Ensure clinically accurate documentation according to policy.
• Review interdisciplinary team progress notes to verify care plan adherence; report concerns to the supervisor.
• Inform nursing personnel of new admissions and resident care needs; prepare rooms for new admissions.
• Notify residents' healthcare practitioners and families of changes in resident condition.
• Coordinate with support personnel (Housekeeping, Dining Services, Nursing Assistants) to plan and monitor shift services and activities.
• Coordinate with the interdisciplinary team to maintain the resident’s total regimen of care.
• Assist in arranging transportation and third-party services for residents.
• Report allegations of misconduct to the facility Risk Manager and Director of Nursing.
• Prepare and administer medications as ordered by healthcare providers.
• Ensure an adequate supply of floor stock medications, supplies, and equipment; report needs to the supervisor.
• Review medication administration records for completeness and accuracy; report concerns to the supervisor.
• Dispose of drugs and narcotics according to established procedures.
• Adjust staffing needs to meet resident care requirements.
• Develop work assignments, schedule duty hours, and supervise nursing staff to ensure performance meets standards.
• Assist in staff development, monitor performance, and apply coaching, counseling, and discipline methods.
• Work with the Director of Nursing to resolve staff performance issues, including terminations.
• Review complaints and grievances, conduct investigations, and develop action plans with the Director of Nursing.
• Participate in planning, conducting, and scheduling orientation and training classes to meet state and federal requirements.
• Maintain clean and sanitary nursing service work areas, resident rooms, and treatment areas.
• Recommend equipment and supply needs to the Director of Nursing; ensure adequate stock levels of medications and supplies.

Requirements

• Ability to make independent decisions, follow instructions, and accept constructive criticism.
• Tactful interaction with personnel, residents, family members, visitors, and government agencies.
• Ability to work with ill, disabled, elderly, and emotionally upset individuals, including potentially hostile situations.
• Proficiency in speaking, writing, and understanding English for effective communication.
• Experience as a supervisor in a hospital, long-term care facility, or related healthcare setting preferred.
• Experience in supervision, teaching principles, and practices.
• Strong leadership, communication, creativity, integrity, and initiative.
• Critical thinking skills to address clinical issues.
• Active LPN license in the state of employment.

Schedule

Full Time, Monday-Friday 2pm-10pm, with on-call weekend rotation.

Working Conditions and Physical Demands

• Potential and/or direct exposure to blood, body fluids, infectious diseases, air contaminants, and hazardous chemicals.
• Physical activities include walking long distances, standing for extended periods, and sustaining repetitive movements (e.g., CPR compressions).
• Ability to stand, walk, and work on your feet for 8-12 hours with minimal breaks.
• Ability to lift and push/pull up to fifty pounds at times.
• Cognitive requirements: execute tasks independently, express ideas clearly, and demonstrate general computer literacy.
• Exposure to ambient room temperatures and lighting.
