LPN - PRN - Home Health

Job Summary This LPN role involves providing PRN home health care primarily for post-hospital patients with higher acuity needs, requiring at least one year of med-surg or relevant experience. The LPN will travel within a designated territory south of Douglasville, covering areas such as Cobb and Cherokee counties, and work under RN or manager supervision within the LPN scope of practice. Key responsibilities include assessing and managing patient needs according to physician ordersperforming routine nursing procedurescoordinating care with team memberseducating patients and caregivers to promote self-caredocumenting care accurately and timelyparticipating in staff education, meetings, and special projects The role requires minimum certifications in BLS (Basic Life Support) and LPN licensure , with a preference for candidates with experience in high-acuity home care.
