LPN - PRN - Home Health

Role Overview: An experienced Licensed Practical Nurse (LPN) providing home health care, primarily for post-hospital patients with varying acuity levels. This PRN position requires a minimum availability of two shifts per month, with flexibility to meet community needs. Responsibilities Assess and manage patient needs following physician orders under RN supervision.Perform routine nursing procedures and deliver age-appropriate care addressing physical, emotional, and psychosocial needs.Coordinate care with the healthcare team, update on patient condition changes, and assist in developing care plans.Educate patients and caregivers on self-care and support systems.Maintain accurate documentation and participate in team meetings, in-services, and continuing education.Contribute to staff training and serve as a preceptor if needed.Participate in call rotations and weekend visits as required. Qualifications & Skills High school diploma or GED, with valid LPN license and BLS certification.Minimum of one year of medical/surgical nursing experience in an acute care setting preferred.Strong communication, documentation, and teamwork skills.Ability to work independently and adapt to varying patient acuity levels.
