LPN - PRN - Home Health

About the role

Licensed Practical Nurse (LPN) providing home health care on a PRN basis, primarily serving post-hospital patients with varying acuity levels. This role offers an opportunity to provide compassionate, high-quality care in patients' homes, supporting recovery and wellbeing within a collaborative healthcare environment.

Responsibilities

• Assess and manage patient needs following physician orders under RN supervision.
• Perform routine nursing procedures and coordinate care with team members.
• Educate patients and caregivers to promote self-care and support.
• Document care accurately and participate in team conferences.
• Contribute to staff education, participate in in-services, and stay current in clinical knowledge.
• Serve as a preceptor for new staff and support community wellbeing initiatives.

Qualifications & Skills

• High school diploma or GED with a valid LPN license and BLS certification.
• At least one year of medical/surgical nursing experience in an acute care setting preferred.
• Strong communication, documentation, and teamwork skills.
• Flexibility to travel within assigned territory and availability for call and weekend visits.
