LPN Private Duty

Responsibilities

• Provide direct nursing care to acutely and chronically ill patients in their homes, following physician prescriptions and nursing scope of practice.
• Document patient visits thoroughly in electronic medical records and electronic visit verification systems.
• Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, physicians, and team members.

Qualifications & Skills

• Current, unrestricted LPN licensure in the practice state.
• CPR certification, valid driver’s license, vehicle insurance, or access to reliable transportation.
• Ability to work independently, manage multiple tasks, and adapt to diverse home environments.
• Preferred: home care experience, flexible hours, strong communication, organizational, and writing skills.
