About the Role

The Home and Community Based Services Licensed Practical Nurse (LPN) provides direct nursing care to acutely or chronically ill patients in their homes, under the supervision of a Registered Nurse or supervisor.

Responsibilities

• Deliver high-quality clinical services within legal scope, following care plans and infection control procedures.
• Document patient visits thoroughly in electronic medical records and visit verification systems.
• Participate in care coordination, reporting patient needs and changes, and communicating effectively with patients, families, physicians, and team members.

Requirements

• Current, unrestricted LPN licensure and CPR certification.
• Valid driver’s license, vehicle insurance, or access to transportation.
• Ability to work independently and adapt to various home environments, demonstrating good communication, organizational, and multitasking skills.

Preferred Qualifications

• Home care experience.
• Flexible hours and strong organizational skills.

Pay

Hourly rate ranging from $22.90 to $34.35.
