About the role
Licensed Practice Nurse (LPN) providing in-home patient care with supervision from a clinical manager.
Responsibilities
- Assess, plan, and implement personalized care
- Educate patients and families on disease management, procedures, and medications
- Manage wound care, tracheostomy, G-tubes, catheters, ventilators, and medication administration
- Develop customized care plans based on patient needs and family requests
- Deliver quality, compassionate care
- Build long-term patient relationships
- Support patient independence and health goals
- Collaborate with healthcare team
- Utilize electronic charting
- Adapt to case diversity and acuity levels
Qualifications
- Valid LPN license
- Excellent clinical skills
- Ability to manage diverse cases including pediatric and adult patients
- Strong communication and problem-solving abilities
- Willingness to participate in training programs
