Provides direct nursing care to acutely and chronically ill patients in their homes, under supervision of a Registered Nurse or supervisor.
Responsibilities
- Deliver high-quality clinical services per the plan of care
- Document patient visits accurately in electronic medical records
- Participate in care coordination by reporting patient needs and changes
- Attend case conferences
- Communicate effectively with patients, families, and healthcare team members
Requirements
- Current unrestricted LPN license
- CPR certification
- Valid driver’s license
- Access to transportation
