About the role
Provides quality patient care and skilled technical services in patients' homes, adhering to state laws and the plan of care.
Responsibilities
- Deliver clinical services within the scope of practice
- Thorough documentation of patient visits in electronic records
- Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, and healthcare team members
Requirements
- Current unrestricted LPN license
- CPR certification
- Valid driver’s license, dependable vehicle or transportation
- Ability to work flexibly and adapt to various home environments
