About the Role

The Home and Community Based Services Licensed Practical Nurse provides direct nursing care in patients' homes, following physician prescriptions and within the scope of practice defined by state laws.

Responsibilities

• Deliver high-quality clinical services in patients' homes.
• Document patient visits accurately and coordinate care activities with the healthcare team.
• Report patient needs and changes in condition promptly.
• Attend case conferences and communicate effectively with patients, families, physicians, and team members.
• Maintain compliance with infection control procedures and regulatory requirements.

Requirements

• Current LPN licensure in the state.
• CPR certification.
• Valid driver's license with reliable transportation.
• Minimum of one year of LPN experience in the state.
• Ability to work independently and adapt to various home environments.
• Must pass a drug test and meet all legal employment requirements.

Skills

• Good communication and organizational skills.
• Ability to manage multiple tasks and demonstrate flexibility in work hours.
• Preferred experience in home care.

Pay

Compensation ranges from $22.90 to $34.35 per hour.
