Home and Community Based Services Licensed Practical Nurse Role Overview: Provide direct nursing care to acutely or chronically ill patients in their homes, following physician prescriptions and coordinating care activities to ensure high-quality patient outcomes. Work under supervision of a Registered Nurse or designated supervisor. Key Responsibilities Deliver clinical services within the scope of practice, adhering to state laws, care plans, and infection control procedures.Thoroughly document patient visits in electronic medical records and visit verification systems.Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, healthcare providers, and team members. Qualifications Current, unrestricted LPN licensure in the practice state.CPR certification or ability to obtain within 90 days.Valid driver’s license, vehicle insurance, and reliable transportation or access to public transit.Ability to serve diverse patient populations regardless of background or condition. Preferred Skills: Flexibility, multitasking, independence, strong communication, writing, and organizational skills.
