Role: Home and Community Based Services Licensed Practical Nurse (LPN) Responsibilities Include Providing direct nursing care in patients' homes according to physician orders and within the scope of practice.Documenting patient visits thoroughly in electronic medical records and electronic visit verification systems.Participating in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, and healthcare team members.
