About the role
Licensed Practical Nurse (LPN) providing skilled nursing care in a patient's home.
Responsibilities
- Assess, plan, implement, and evaluate patient care using the nursing process.
- Deliver skilled nursing services according to prescribed orders.
- Monitor physiologic and psychosocial changes in the patient’s condition.
- Collect, analyze, and interpret health data, documenting nursing diagnoses and observations.
Qualifications & Skills
- Current LPN/LVN license in the practicing state.
- Valid TB or Chest X-Ray, and BLS certification.
- Preferably at least one year of professional nursing experience.
Key Duties
- Provide patient assessments, deliver appropriate nursing interventions, and monitor patient health to ensure quality care in a home setting.
