LPN Pediatric Home Care Night Shifts

About the Role

Provide direct nursing care in patients' homes per physician’s orders, work under the supervision of a Registered Nurse or designated supervisor, and ensure high-quality clinical services within legal scope.

Responsibilities

• Deliver direct nursing care in patients' homes according to physician’s orders
• Document patient visits thoroughly
• Coordinate care activities and communicate effectively with patients, families, and healthcare team members
• Adhere to infection control procedures
• Participate in care coordination and case conferences

Requirements

• Current, unrestricted LPN licensure
• CPR certification
• Valid driver’s license or access to transportation
• Ability to serve diverse home environments
• For Louisiana: at least 1 year of LPN experience
