LPN Pediatric Home Care Night Shift

About the role

The Home and Community Based Services Licensed Practical Nurse provides direct nursing care to acutely or chronically ill patients in their homes, following physician prescriptions and documenting progress notes. They coordinate patient care activities, work under the supervision of a registered nurse or supervisor, and ensure high-quality clinical services within legal scope and infection control protocols.

Responsibilities

• Provide direct nursing care to patients in their homes
• Follow physician prescriptions and document progress notes
• Coordinate patient care activities
• Ensure compliance with legal scope of practice and infection control protocols
• Maintain thorough documentation in electronic records
• Participate actively in care coordination
• Communicate effectively with patients, families, and healthcare team members

Requirements

• Current, unrestricted LPN license
• CPR certification
• Valid driver’s license and reliable transportation

Qualifications

• Home care experience (preferred)
• Ability to work independently and manage multiple tasks
• Strong communication skills

Pay

$22.90 to $34.35 per hour
