Responsibilities
- Provide direct nursing care to acutely or chronically ill patients in their homes, following physician prescriptions
- Record clinical notes and document patient visits thoroughly in electronic medical records
- Coordinate patient care activities, communicate effectively with patients, families, physicians, and care teams
- Participate in care coordination, report patient needs and changes, and attend case conferences
Qualifications & Skills
- Current unrestricted LPN licensure and CPR certification
- Valid driver’s license, vehicle insurance, or reliable transportation
- Ability to work in diverse home environments and function independently
- Strong communication, organizational, and multitasking skills
Preferred: Home care experience, flexibility, and ability to work independently.
Pay
Ranges from $22.90 to $34.35 per hour
