About the role
The Home and Community Based Services Licensed Practical Nurse provides direct nursing care in patients' homes, following physician prescriptions.
Responsibilities
- Deliver high-quality clinical services within legal and care plan guidelines, adhering to infection control protocols.
- Document patient visits accurately in electronic medical records and electronic visit verification systems.
- Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, and healthcare team members.
Requirements
- Current unrestricted LPN licensure and CPR certification.
- Valid driver's license, dependable vehicle or transportation access.
- Ability to work with diverse populations and adapt to various home environments.
Preferred Qualifications
- Home care experience.
- Strong communication and organizational skills.
- Ability to work independently.
Pay
Compensation ranges from $20.50 to $30.75 per hour.
