LPN Pediatric Home Care Night Shift

About the role

Provide direct nursing care to acutely or chronically ill patients in their homes, documenting progress and coordinating care activities to ensure high-quality patient care. Work under supervision of a Registered Nurse or designated supervisor.

Responsibilities

• Deliver clinical services within the scope of practice, following the care plan and infection control protocols.
• Accurately document patient visits in electronic medical records and visit verification systems.
• Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, healthcare providers, and team members.

Requirements

• Current, unrestricted LPN license and CPR certification.
• Valid driver’s license, vehicle insurance, or access to transportation.
• Ability to work in diverse home environments regardless of background.
• Minimum of one year of LPN experience, with state-specific licensure in LA.

Pay

Compensation is hourly, ranging from $22.90 to $34.35.
