LPN Job Details
- Pay Range: $32 - $38 per hour

LPN Responsibilities
- Assures residents maintain admission criteria, monitor, and recommend alternative placement if necessary.
- Assures residents have up to date care plans per state regulations.
- Maintains resident charts and clinical progress notes.
- Communicates to physicians, staff and family changes in resident’s needs or conditions.
- Facilitates coordination of resident services with other service providers.
- Monitors and maintains resident charts and clinical progress notes.
- Oversees medications and renewals and any necessary follow-up with physicians about medications and/or medical conditions.
- Communicates in resident’s log any information that staff need to be aware of.
- Provides alternating supervisory visits in coordination with the RN.
- Works as a team player with all members of the residents’ care team.
- Supervises medication practices and medical equipment being used in the building.
- Affirms that staff clearly understand and follow all resident care procedures.
- Shares resident and family concerns with pertinent staff if appropriate.
- Works with manager to assure emergency procedures are in place.
Qualifications
- Maintains a current state license as an LPN (Licensed Practical Nurse) per state regulations.
- A minimum of one year's experience in home health, assisted living or long-term care is preferred.
- Good organizational skills with the ability to self-start and follow through with initiatives with a minimal amount of supervision.
- Exhibits a caring heart with a passion to care for the elderly.
