About the Role

We are seeking a Licensed Practical Nurse (LPN) to join our Skilled Nursing team, providing one-on-one, hands-on care to patients ranging from infants to geriatrics in their homes.

Responsibilities

• Travel to client homes to deliver care
• Execute and document doctors' orders
• Perform patient assessments
• Monitor and evaluate clients' conditions
• Build strong client relationships
• Maintain accurate and detailed documentation

Requirements

• Graduation from an accredited nursing program
• Current, in-good-standing nursing license in the state
