About the role
We are seeking a Licensed Practical Nurse (LPN) to provide patient care in a home setting under the supervision of a Registered Nurse (RN).
Responsibilities
- Utilize the nursing process to assess, plan, implement, and evaluate patient care
- Conduct focused interviews to identify patient needs
- Assess signs and symptoms to detect physiologic and psychosocial changes
- Collect, analyze, and interpret healthcare data and document diagnoses
- Perform other assigned LPN duties
Qualifications
- Current LPN license in the practicing state
- Valid health certificate, PPD or chest X-ray, and BLS certification
- One year of LPN experience preferred
