About the Role

We are seeking a Licensed Practical Nurse (LPN) to join a Skilled Nursing team, providing one-on-one, hands-on care to patients ranging from infants to the elderly. This role emphasizes building client relationships, observing milestones, and delivering high-quality care tailored to individual needs. You will travel to client homes to perform assessments, monitor conditions, and follow doctors' orders.

Responsibilities

• Travel to client homes to provide one-on-one, hands-on care
• Follow and document doctors' orders accurately
• Perform patient assessments and monitor client conditions
• Build strong client relationships and deliver individualized care

Requirements

• Graduation from an accredited nursing program
• Current, valid nursing license in good standing
