LPN Medical Care Manager

About the role

An LPN Medication Nurse provides licensed nursing support in senior care settings, ensuring safe medication administration, health monitoring, and accurate clinical documentation.

Responsibilities

• Administer medications and treatments following physician orders and the Six Rights of Medication Administration
• Document accurately in MAR/TAR in real-time
• Monitor residents for symptoms, adverse reactions, and health changes
• Transcribe physician orders and support care plan updates
• Report refusals, adverse reactions, and physical, emotional, or cognitive changes promptly
• Maintain medication carts, perform narcotic counts, and ensure compliance with regulations

Qualifications & Skills

• Active LPN license in good standing
• CPR and First Aid certifications
• Strong assessment and documentation skills
• Experience in senior care or medication administration is preferred
